DOCUMENTS REQUIRED FOR ADMISSION

In order to expedite the application process, photocopies of all documents required should be prepared in advance for school records. *** Applications without the required documents will be considered incomplete and will not be processed until all documents are submitted.***

Pre-Kindergarten 3:
A child must be 3 years of age on or before September 30 th . Copies of the following documents are required:
•  Certificate of Baptism
•  Official State Birth Certificate
•  Immunization Record: This card should be made available to you from your doctor or the Health Unit.
•  Student's Social Security Card

Pre-Kindergarten 4:
A child must be 4 years of age on or before September 30 th . Copies of the following documents are required:
•  Certificate of Baptism
•  Official State Birth Certificate
•  Immunization Record: This card should be made available to you from your doctor or the Health Unit. A single dose of HIB vaccine and a single dose of MMR vaccine is required from all children entering Pre-Kindergarten with a second dose at or after age 4.
•  Student's Social Security Card

Kindergarten:
A child must be 5 years of age on or before September 30 th . Copies of the following documents are required:
•  Certificate of Baptism
•  Official State Birth Certificate
•  Student's Social Security Card
•  Immunization Record: This card should be made available to you from your doctor or the Health Unit.

Immunization Required:
•  Minimum of 4 DPT immunizations
•  3 doses polio vaccine – last dose after 4 years of age
•  2 doses of MMR vaccine (measles, mumps & rubella) with last dose at or after 4 years of age
•  Varivax vaccine

New Students Grades 1-6 : Copies of the following documents are required:
•  Certificate of Baptism
•  Official State Birth Certificate
•  Immunization Card
•  Student's Social Security Card
•  Most recent report card and Achievement Test scores
•  Any educational or psychological evaluation administered to your child
